New Client Intake

Becoming a client of Body Positive and taking advantage of our full range of services is easy. We offer wellness, nutrition, behavioral health, support groups, education, family and other services, each tailored to meet your individual needs.

HIV+ people who live in Maricopa or Pinal County can become a client of Body Positive by following these easy steps

1. Download an intake packet below.
2. Call or stop by the Body Positive office and request an intake packet from the Medical Records Specialist.
3. Return the intake packet with the requested eligibility documents to our Medical Records Specialist and schedule an intake appointment.

At the one-hour intake appointment, a licensed Behavioral Health therapist or technician will meet with you to review your paperwork and explain the variety of services we offer. We will point you in the right direction and help you get you the services you will most benefit from.
